Job Description

Summary:

The Community Health Worker (CHW) will work on two Brown Health programs under their respective supervisors the Mi Gente Program and the Connect for Health Program in the Community Health Institute. Due to the demographics of the patients and families we serve Spanish language proficiency is required .

The CHW will assist Mi Gente Program on a clinical research study aim to test a model of providing treatment to Latine/Hispanic youth who experience suicidal thoughts and behavior and their caregivers. An affirmative and culturally relevant treatment will be provided to all youth and half of the families will be assigned to the additional support of a community health worker (CHW). The CHW will be trained to educate on topics related to mental health self-care Evidence-Based Treatments and LGBTQ+ diversity among others. The CHW will support family engagement in treatment (e.g. calling families to assess barriers to come to treatment) and assess each family�s social needs and concerns about treatment. In addition the CHW will support the planning organizing and carrying out of mental health awareness activities such as Wellness Fairs to create mental health awareness in the community. The CHW will also provide support on administrative tasks related to the ongoing functioning of the Mi Gente clinical program.

Connect for Health (C4H) screens patients for health-related social needs and provides navigation support to secure basic needs. In doing so C4H uniquely bridges medical community and social needs to help patients achieve a complete state of health. Starting more than 20 years ago as a family help desk with a handful of volunteers C4H has evolved into a program with 175+ Advocates who serve thousands of patients a year. C4H is a program of Brown University Health's Community Health Institute (CHI) which works to eliminate health disparities and promote health equity. The CHI and C4H manifest Lifespan�s mission Delivering health with care by not only providing medical treatment when patients are sick but also addressing the social determinants of poor health. 

Responsibilities:

�� Works with clients in their homes and communities to help manage their health navigating systems providing care coordination and coaching in a variety of behavioral health issues.

� Uses a multi-disciplinary team approach to address opportunities to plan and coordinate care.

� Uses Motivational Interviewing skills and other client engagement techniques with clients family members and caregivers.

� Assists in the development of the client�s Treatment Plan to include goals and actions designed to improve the client�s functioning and remove the barriers that are preventing them from gaining access to high quality and timely primary and/or behavioral health care.

� Uses required data collection and reporting strategies to understand client needs and prioritize follow-up.

� Identifies and uses culturally sensitive approaches and community resources.

� Builds upon the relationship between Community Health Workers and providers in the state through intentional outreach and engagement efforts (i.e. canvasing meetings hosting community events).

� Advocates for the socioeconomic environmental and political rights of the population served.

� Uses a variety of online platforms to connect clients to resources and community partners.

� Gets information out to the community by disseminating program-related information supporting events and making referrals.

� Performs additional responsibilities consistent with initiative needs and other duties as assigned.

� This role will be partially (50%) dedicated to supporting the Connect for Health program at the Center for Primary Care. This CHW will work directly with patients experiencing barriers to care and will help support advocates (student volunteers). The CHW will be a critical member of the care team enabling coordination between program management clinic staff advocates and community partners.

QUALIFICATIONS:

� Highschool or GED required BA/BS degree preferred

� Bilingual (Spanish & English)

� Community Health Workers certification preferred; non-certified incumbents are expected to earn certification within 6 months of hire date.

� Personal experience with navigating and supporting physical and behavioral health issues valued.

� Able to complete necessary certifications such as Motivational Interviewing Basic Training on the Protection of Human Subject in Research etc.

� Excellent organizational skills to manage multiple priorities and tasks.

� Valid driver�s license and reliable transportation are required.

� High level of interpersonal skills to engage with clients and community partners.

� Demonstrated proficiency with Microsoft Office/computer skills to enter data prepare reports and correspondence.

� Observe all agency policies and procedures.

� Strong collaboration and coordination of care skills.

� Ability to understand embrace and reinforce agency ethics and codes of conduct in all contacts with clients coworkers and the community.
